Approval to Work Application

Please be informed of the following requirements when submitting this form:

  • A State Contractors License is required.
  • Applicants must submit a certificate of insurance for a minimum of $1,000,000 general liability and MUST list Town of Gilbert as the certificate holder.
  • These requests will NOT be processed unless the certificate of insurance is provided.
  • Excavation & Construction work will NOT be permitted in public right-of-way or public utility easements on Saturdays,Sundays or legal holidays.
  • IF working on or near a roadway, you must also complete a Traffic Engineering Application (separately).
  • If you have multiple permits; a separate Approval to work application must be submitted along with certificate of insurances for each.
  • Please allow up to 48 hours of processing of your request.
